SR & JR High Registration ('25-'26)

Crossroads Senior and Junior High Student Ministry meets weekly on Wednesday nights throughout the school year from 6:30-8:00pm. We need an updated parent authorization/registration on file for every student grades 6-12 for the 2025-26 school year.

**This authorization/registration MUST be filled out by a parent.
